The Path Project:

The Change Foundation (TCF) released its latest publication, Charting a New PATH – a three-part report that describes what TCF has learned as participants in, and funder of, the Partners Advancing Transitions in Healthcare (PATH) project. As you know, PATH was one of two showcase engagement projects funding under our Hearing the Stories, Changing the Story strategic plan. Bereavement Ontario Network - is a diverse group of organizations and individuals throughout the province that work in the field of grief, bereavement, and mourning as professionals and volunteers.

Community Care Access Centre- Connectors to home care, long-term care destinations, and other services in your community.

Hospice Palliative Care Ontario - provides information on community based, volunteer organizations providing end-of-life care for individuals living with a life-threatening illness, and those who care for them.
