This program provides supports which include any services, support, equipment or retrofit needed by an individual, or their caregiver, that cannot be accessed in a timely manner through an existing service provider.  Examples are personal support assistance, homemaking services, retrofits, speech therapy or occupational therapy etc. These services can last for a few weeks to a few months depending on the initial referral, frequency of support needed and assessment of client at follow-up.

Eligibility criteria:   

• • Northumberland Hills Hospital (NHH) patients who could be safely discharged sooner if additional support, service, equipment or retrofit is made readily available upon discharge from the hospital and cannot pay privately or does not qualify for financial assistance.

• • NHH patients who could be safely discharged sooner if additional support, service, equipment or retrofit is made readily available upon discharge at the patient’s cost but coordination of these services is required

• • NHH patients who have visited the ER department and could be safely diverted from admission with additional support
